	Visit one of Congressman Murtha's Outreach Offices

Mail should be sent to the main District Office in Johnstown.  Please do not send mail to Congressman Murtha's Washington, D.C. Office or any Outreach Office unless specifically directed to do so.  Mail deliveries to the D.C. Office are screened for protection from biological or other potentially hazardous substances, which delay delivery by several weeks.  Because the 12th Congressional District is spread out over parts of nine counties, outreach offices are not continually staffed, so delays of at least a few days and sometimes up to a few weeks occur when mail is sent to an outreach office.

For assistance with federal agencies, please submit the following information to enable my staff to review your case and contact the appropriate federal agencies on your behalf:

1. A clear and concise letter describing the matter about which you request help, including a daytime phone number and address in case my staff needs additional information from you.

2. Also, be sure to include any identification numbers such as your social security number, date of birth and case or file numbers.

3. A completed Privacy Act Release Form to grant your written consent for my office to contact a federal agency on your behalf.
